Patient Forms

These forms have been designed to enable patients to securely submit information to the practice.

They are emailed to the practice and passed along to the relevant staff member to process.

These forms are only received at the practice during office hours (Monday to Friday 9am to 3.30pm) are not processed urgently, so if you do have any immediate health concerns, please telephone the practice in the usual way by calling 0113 2537628 and do not use any of these forms for that.

GP Appointment request / query:

This route to contact the surgery is strictly for non urgent problems only, no need to provide specific medical information at this stage. If you already know that you do need medical care or advice, you should contact the surgery in the usual way by telephoning 0113 2537628.

If you need urgent emergency care you must phone 999 now.

If the practice is closed, and you urgently need to speak to a GP, you should phone 111

Travel

If you wish to book in for travel vaccinations at Leigh View Medical Practice, please complete the enclosed form which is emailed to our Nursing Team.

You shall then be contacted once the Nurses have reviewed this, and an appointment will then be booked if you need one.

Asthma

If you have asthma and have been asked to submit a questionnaire to determine if you need to come in, please complete the enclosed form which is emailed to our Nursing Team.

If you are unwell, do not wait for this to be processed, instead please book an appointment for a review.
