REGISTER WITH THE PRACTICE?
When you request to join the practice
list you will be asked for your permanent address details; provided you reside within
the practice area you will be asked to complete a GMS1 form. This form will ask
for your full name, permanent address details, place of birth, previous permanent address and your previous GP name/address. Please take
time to make sure all these details are entered as this enables the practice to
help the Health Authority trace your Lloyd George medical records.
The Health Authority no longer issues a medical card.
New Patient Health Checks
As a newly-registered patient you will be asked to complete a health questionnaire
and attend for an appointment with the healthcare assistant.
Staying for a Short Time in the Area
If someone is staying with you for a short time and becomes ill, they can obtain medical care from the practice provided the address is still within the practice boundary. He/she will be asked to complete a temporary resident form.
Similarly, if you go on holiday in the United Kingdom and become ill whilst away from home,
you can also register as a temporary patient at your holiday venue.
Students living away from home whilst attending college/university should register
with a doctor in the area where they spend most of their time.
Change of Name / Address / Telephone Numbers
If you change your name, move house or change telephone numbers please let us have your correct details in writing as soon as possible.
Moving Outside the Practice Area
If you move outside the practice area you will need to register with a GP practice that is local to your new address. If you need help:
