COMPLAINTS

If you are unhappy or wish to make a comment on the service provided for you by the practice, you can speak to the practice office manager or write a letter. In the first instance the practice will acknowledge your complaint by return of post. A full investigation will be carried out and a written reply will follow.

A meeting may be offered to discuss the complaint.

The patient’s representative can make a complaint with the written consent of the patient.

FACILITIES FOR THE DISABLED

Leigh View Surgery has been designed to allow ease of access for wheelchair-bound patients, including special toilet facilities.

We also offer a ‘loop’ facility for our patients who are hard of hearing.

BOOKING HOSPITAL TRANSPORT - 0113 253 7628/9

We can only book hospital transport for your first clinic appointment if there is a medical need for transport. We need a minimum of 48 hours before your hospital visit to book transport.
Please do not abuse this service; it is a very expensive strain on the NHS and is ONLY intended for patients who cannot go to the hospital unaided.

Please do NOT ask to take an escort/partner with you, unless it is medically necessary.
