OBTAIN REPEAT
PRESCRIPTIONS?
Please allow two working days for
routine repeat medication requests.
If you add any item(s) to your repeat request, or there have been any changes
by a hospital consultant, we will need to check with the doctor before these can
be issued. Please allow more time to process these requests.
You can bring your request to the surgery or post it (please enclose stamped, addressed
envelope if you wish your prescription to be posted back to you allow more time
for the postal service).
Please do NOT fax your request as these have often proved difficult
to read.
Please do NOT telephone your request either. This is for security
reasons and to ensure accuracy. Mistakes can have serious consequences.

Certain medical conditions entitle patients to free prescriptions - your GP will inform you if you meet this criterion.
You can purchase a prescription charge 'season ticket' from the prescription pricing authority; please ask for a form at the pharmacy.

The Partnership wishes to encourage continuity of care as best practice and feels your long term medical needs are best served by seeing the same GP. You will appreciate that this is not ALWAYS possible but every effort is made to comply with patients needs within the constraints of the appointment system. The receptionists are here to help you and may offer you a choice of options:

for a booked appointment PLEASE telephone the surgery to cancel it.
Over 100 patients miss appointments every month. If you telephone early, your
appointment can be offered to someone else. We appreciate your call and so will
another patient.
CHOOSING

Booked
Appointment (10 min)
Offers a choice of time and GP. It is a consultation to discuss your long term
or new medical problems. We encourage continuity of care with your usual GP.
Advanced Access Clinic
This Clinic is available Monday to Friday mornings and offers a shorter appointment
for medical symptoms that may need treatment or reassurance on a one-off basis.
It is NOT for review of long-term/ongoing medical problems.
Telephone Advice
If you feel your medical problem could be dealt with over the telephone, the doctors/practice nurses are happy to call you. Please give the receptionist an appropriate telephone number.
Urgent Appointments
PLEASE ONLY REQUEST AN URGENT APPOINTMENT IF IT REALLY IS MEDICALLY NECESSARY.

If you are not well enough to come
to surgery you can ask for a home visit. Please telephone before 10.30am if you
can.

Please ONLY request a home visit if you are elderly and housebound because of
a medical problem.
Lack of transport is NOT a reason to request a home visit.

OBTAIN TEST RESULTS? -

Please allow five working days before
you ask for your result (10 working days for x-ray and ultrasound results).
The doctors review the results before they are made available to patients.
Staff cannot enter into any clinical discussion with you and, for reasons of confidentiality,
will only give results directly to the patient.
Please do not ask a relative or friend to telephone for your result.

REGISTER WITH THE PRACTICE?
When you request to join the practice
list you will be asked for your permanent address details; provided you reside within
the practice area you will be asked to complete a GMS1 form. This form will ask
for your full name, permanent address details, place of birth, previous
permanent address and your previous GP name/address. please take
time to make sure all these details are entered as this enables the practice to
help the Health Authority trace your Lloyd George medical records.
The Health Authority no longer issues a medical card.
New Patient Health Checks
As a newly-registered patient you will be asked to complete a health questionnaire
and attend for an appointment with the healthcare assistant.
Staying for a Short Time in the Area
If someone is staying with you for a short time and becomes ill, they can obtain medical care from the practice provided the address is still within the practice boundary. He/she will be asked to complete a temporary resident form.
Similarly, if you go on holiday in the United Kingdom and become ill whilst away from home,
you can also register as a temporary patient at your holiday venue.
Students living away from home whilst attending college/university should register
with a doctor in the area where they spend most of their time.
Change of Name / Address / Telephone Numbers
If you change your name, move house or change telephone numbers please let us have your correct details in writing as soon as possible.
Moving Outside the Practice Area
If you move outside the practice area you will need to register with a GP practice that is local to your new address. If you need help:
