OBTAIN HOME VISITS? - 0113 253 7629/253 7628

If you are not well enough to come to surgery you can ask for a home visit. Please telephone before 10.30am if you can.

Give the telephonist:

your name
address
telephone number
medical symptoms.

This information will help the doctor to decide whether a home visit is necessary and how urgent it is.

Please ONLY request a home visit if you are elderly and housebound because of a medical problem.
Lack of transport is NOT a reason to request a home visit.
